I am new here. Sorry if this topic or similar has already been discussed, I have failed at finding it.
My car is the European version of 3.0R spec B legacy, 2007 (facelift). The engine is of course EZ30D with AVCS and AVLS. ECU ID = D2UH001L / CAL ID = 4B4A384007.
As customary, the car has received a set of custom made headers and full stainless exhaust system (but still retains all three OE catalysts). I have also remapped the ECU to adjust the target AFR maps, DBW maps and ignition maps. Power-wise, it runs great, but over time it has also developed a few bugs.
1) First of all is a completely failed emissions test with CO and HC values exceeding the limit three-fold, forcing me to resort to E85 just to pass the yearly technical inspection, but just barely. The exhaust visibly smokes and smells like raw gasoline. The head gaskets are fine and there is little oil consumption. Mechanically the engine is healthy and overall the fuel consumption is great for this engine.
2) When the engine is cold, driving under light throttle, it jerks for a brief moment between 1000 and 2000 RPM, this eventually becomes less noticeable as the engine heats up but never fully goes away. It is as if during the climb from, say, 1300 to 1600 RPM, the torque oscillates 2 - 4 times. It does not happen under mid to heavy throttle, only light.
3) During cruising, I have noticed the fuel-consumption gauge is not stable and keeps oscillating to some degree, and after doing some data logging I can see that all fuel related parameters are oscillating seemingly for no reason, as if it keeps hunting for the perfect AFR but always overshoots and then overpulls the correction.
4) At some stage after a fresh reflash I noticed that the engine would not run steady while cruising, but would outright cut out for a brief moment and jerk violently. Unfortunatelly, I wasn't data logging during that time so I have no idea what was causing it, but the issue went away after I reflashed a different version of the same ROM. I wrote it off as just a bug during reflashing, but I really have no idea.

I have attached my tuning file and one of the data logs to illustrate the problem. Would any of you care to point me in the right direction? At this point I am completely stumped by these issues and I am looking for some advice. I thank you for your time.

Could you share a WOT pull data log and the Learning Table View? At least IAM is at 1 so may not be an ignition timing issue.

I don't think these are ROM related issues.
What's your AFR sensor? They should be new Denso according to the catalog, and nothing else.
If they're new, you can check your AFR on the go using the program BtSSM. It's very convenient. They have site with latest version for Android.
If your mixture is normal, i.e., close to 14.7 or even slightly rich, there shouldn't be any gasoline smell.
Most likely, your AFR sensors are malfunctioning, and the computer is overfilling the fuel mixture.
Next, you need to make sure there's no air leak in the intake manifold; you'll need a smoke generator for that.
Next, clean the throttle body.
Next, if the car jerks at low RPMs during acceleration, it's 99% the spark plugs and coils. Even if there are no ignition errors, a worn spark plug and rusty coil will produce a weak spark.
I replaced all the spark plugs with NGK iridium from the catalog and a set of aftermarket spark plugs, and the car ran like new.
